Referrals

We welcome referrals for surgeries and second opinions at Centre Animal Hospital.

Please use the following protocol when sending clients to us:

  1. Complete the referral form below. In the Remarks field, be sure to note if this is a time-sensitive medical issue and when it might need to be addressed.

  2. Send the patient's complete medical record *with the referral form*.

  3. If you have not received a call from us to take your referral appointment fee within 2 days, you must call us to confirm your appointment and pay the reservation fee, or your appointment cannot be guaranteed. This fee will be applied to your post-visit invoice. In the event that you cancel with less than 24 hours' notice or fail to show for your appointment, this fee cannot be refunded.
